Maplewood Care is a 203 bed skilled nursing facility providing a variety of services ranging from 24-hour skilled nursing care, individualized therapies, Alzheimer’s/Dementia Care, Hospice Care, Respite Care and specialized services for the chronically mentally ill.

Maplewood Care is located at 50 N. Jane Drive Elgin, IL 60123. Elgin is home to many diverse cultures and Maplewood Care welcomes diversity. We are a nursing home offering to our patient’s/resident’s a variety of healthcare options to enhance whatever stage of life they may be in.

We are licensed by the State of Illinois and are approved for Medicare, Medicaid, Medicaid Pending and many private insurances.

Maplewood Care prides in giving excellent medical, psychiatric, social and rehabilitation services. We also take pride in the caring we give to each individual that resides in our home.

We treat a variety of mental illnesses in our residential housing setting, including Schizo-Affective Disorder, Major Depression, Bipolar Disorder, Schizophrenia , as well as those who are dually diagnosed with a mental illness and substance abuse. 

Our psycho-social rehabilitation programs include anger management, substance abuse & med management.
